·         Patient’s photograph (Only on MAR chart)

·         Patient’s date of birth

·         Patient’s address

·         Medication’s name

·         Medication’s strength

·         Medication’s dose

·         Medication’s time or interval

·         Medication’s route

·         Allergy advice
